APPOINTMENTS/PAYMENTS

Patients are seen by appointment only, appointments can be made by telephone between 8:00 a.m. and 4:30 p.m. Phone numbers are listed under CONTACT US. The appointment secretary will ask specific questions concerning the nature of your problem in order to allow sufficient time in the schedule for your appointment. Every effort will be made to schedule your appointment as timely and conveniently as possible.

First time patients to the practice are asked to arrive 15 minutes prior to your scheduled appointment. This will allow you ample time to complete the registration process.

FINANCIAL INFORMATION

Charges for medical service are due and payable at time of service, unless prior arrangements have been made. We accept personal checks, cash, MasterCard or Visa. We accept Medicare assignment and participate in several health insurance plans. If you have health insurance, please remember that your health insurance policy is an agreement between you and an insurance company to pay certain amounts for medical care. Your doctor bill is an agreement between you and your doctor. You are responsible for the payment of your bill regardless of the status of your claim.

We recommend that you take the time to review your health care coverage and acquaint yourself with all specific requirements. Many policies and managed care plans now require written referral, pre-admission authorization or other stipulations in order to assure coverage of medical services. Please direct all your questions regarding financial arrangements and insurance to the business office.

MEDICAL FEES

Charges are based on the procedures required and the amount of professional skill and time involved in making a diagnosis and providing treatment. Fees for professional services at OST are in accord with those for specialists practicing throughout the East Texas area.

PRESCRIPTION REFILLS

All requests for prescription refills must be authorized by your doctor. To insure that you receive your refill in a timely manner, please call before you use all of your prescription. Prescription refills must be called in to the office by 4:00 p.m.

CONFIDENTIALITY

Your medical records are maintained in strict confidence and will not be released without your written permission.

SMOKING, EATING AND DRINKING

Smoking is not allowed in OST clinics. We also prefer no food or beverages in the clinics.
